The two most common legal options, and how they differ on cost and coverage.
Esketamine nasal spray for treatment-resistant depression, given only at REMS-certified clinics with at least two hours of monitoring. Usually covered by insurance with prior authorization.
IV ketamine prescribed by licensed clinicians for depression and other conditions. Rarely covered by insurance, so most people pay out of pocket.

Most of Arizona’s ketamine and Spravato clinics are in the Phoenix metro area, spread across Phoenix, Scottsdale, Mesa, Tempe, Chandler, Gilbert, Glendale, and Peoria.
More choice is good news, but it also means more to compare: whether a clinic is in your insurance network, whether it offers Spravato, IV ketamine, or both, how soon it can see you, and how clearly it explains pricing.
Spravato usually starts with two sessions a week for the first month, so a clinic close to home or work makes the schedule much easier to keep. A Same Team navigator can help you narrow the list based on your insurance, location, budget, and preferences.

Clear answers to the questions people ask before choosing a clinic.
Spravato is only given at REMS-certified clinics, and many are located across the Phoenix metro area, including Scottsdale, Mesa, Tempe, Gilbert, and Peoria. You can use the official Spravato treatment center finder at spravato.com, ask your psychiatrist, or schedule a free navigation call and we’ll help you find a clinic that takes your insurance.
IV ketamine infusions in Arizona commonly cost about $300–$800 per session and are usually paid out of pocket. An initial series of around six infusions often totals $2,000–$4,000. Spravato is different: it’s FDA-approved and usually covered by insurance with prior authorization.
AHCCCS rarely covers off-label IV ketamine. AHCCCS health plans may cover Spravato for eligible patients with prior authorization. Our navigation call is free for AHCCCS members; Same Team’s separate coaching services are not covered by AHCCCS.
Some clinics accept self-referrals for an evaluation; others ask for a referral from your psychiatrist or primary care provider. For Spravato, your insurer will need records of the depression treatments you’ve already tried.
Schedule a navigation call and a Same Team navigator will help you understand your options and find care that fits.
