The short answer

Spravato (esketamine) nasal spray is FDA-approved for adults with treatment-resistant depression, and for major depressive disorder with acute suicidal thoughts or behavior. Aetna requires precertification for all Spravato requests.

If you meet Aetna’s criteria, your clinic submits the request and treatment begins after approval. Because Spravato is given in the clinic, choosing a certified clinic that is in network with your plan matters.

What Aetna looks for (treatment-resistant depression)

Summarized from Aetna’s Spravato policy. Your plan’s documents are the final word.

  • Age 18 or older with severe major depressive disorder, documented with a standardized rating scale (such as PHQ-9, BDI, HDRS, or MADRS)
  • An inadequate response to at least two antidepressants from different classes, each taken at the maximally tolerated dose for at least eight weeks, within the past five years
  • An inadequate response to augmentation therapy or evidence-based psychotherapy (such as CBT) during the current episode
  • Prescribed by, or in consultation with, a psychiatrist
  • Given under direct supervision at a certified clinic, with at least two hours of monitoring after each dose
  • No untreated moderate or severe substance or alcohol use disorder

Different criteria apply to depression with acute suicidal thoughts. If you are in crisis, call or text 988.

How to get started

Step 1

Find an in-network clinic

Choose a REMS-certified Spravato clinic that is in network with your Aetna plan. In-network care usually keeps costs lower.

Step 2

Gather your history

List the antidepressants you’ve tried, doses, how long you took them, and any therapy or augmentation you’ve had.

Step 3

Clinic requests precertification

The clinic submits your records to Aetna. Treatment starts after approval.

Step 4

Reauthorization

For treatment-resistant depression, Aetna can continue coverage in six-month periods when rating scales show improvement.

Aetna & Spravato questions

Does Aetna cover Spravato in Arizona?

Aetna commercial plans generally cover Spravato for eligible adults, and Aetna requires precertification (prior authorization) for every Spravato request. Your exact costs depend on your plan’s deductible, copay, or coinsurance.

How many antidepressants do I need to have tried for Aetna to approve Spravato?

For treatment-resistant depression, Aetna’s policy asks for an inadequate response to at least two antidepressants from different classes, each for at least eight weeks at the maximally tolerated dose within the past five years, plus a trial of augmentation therapy or evidence-based psychotherapy.

How long does Aetna approve Spravato for?

For treatment-resistant depression, continued coverage can be approved for six months at a time when standardized rating scales show improvement. For depression with acute suicidal thoughts, use beyond four weeks requires meeting the initial criteria again.

What if Aetna denies my Spravato request?

Your clinic can usually appeal with additional records or a letter of medical necessity. A Same Team navigator can help you understand your options.
