The short answer

UnitedHealthcare’s commercial medical benefit drug policy considers Spravato (esketamine) medically necessary for treatment-resistant depression and for major depressive disorder with acute suicidal thoughts or behavior, with prior authorization.

Approvals are granted for up to 12 months at a time. Because Spravato is given in the clinic, it’s typically billed under the medical benefit, so choosing an in-network clinic matters.

What UnitedHealthcare looks for (treatment-resistant depression)

Summarized from UnitedHealthcare’s commercial Spravato policy. Your plan’s documents are the final word.

  • A diagnosis of treatment-resistant major depressive disorder from a mental health professional
  • No meaningful improvement after trials of at least two different antidepressants or treatment regimens
  • A baseline score on a validated depression scale (such as PHQ-9, BDI, HAMD, MADRS, or QIDS)
  • Prescribed by, or in consultation with, a psychiatrist
  • Given at a REMS-certified clinic

Different criteria apply to depression with acute suicidal thoughts. If you are in crisis, call or text 988.
