The short answer

Tricare covers Spravato (esketamine) for its FDA-approved uses: treatment-resistant depression and major depressive disorder with suicidal thoughts or behavior. Prior authorization is required for every plan type.

Arizona is in the Tricare West Region, administered by TriWest Healthcare Alliance. IV or intramuscular ketamine for mental health and compounded ketamine are not covered by Tricare.

What Tricare looks for

Summarized from TriWest’s Spravato requirements. Confirm your plan’s rules before starting.

  • Age 18 or older with an FDA-approved diagnosis for Spravato
  • An inadequate response to at least two antidepressants at appropriate doses and durations
  • A prescriber and clinic enrolled in the Spravato REMS program
  • Prior authorization submitted with TriWest’s esketamine referral checklist

Different criteria apply to depression with acute suicidal thoughts. If you are in crisis, call or text 988.
