The short answer

Blue Cross Blue Shield of Arizona and other Blue plans generally cover Spravato (esketamine) for eligible adults with treatment-resistant depression, or major depressive disorder with acute suicidal thoughts. Prior authorization is required.

Exact criteria vary by Blue plan, including employer plans administered by an out-of-state Blue. The member services number on the back of your card can confirm your benefits, deductible, and copay.

What Blue Cross plans typically look for

Common requirements across Blue Cross policies. Your plan’s documents are the final word.

  • Age 18 or older with a diagnosis of treatment-resistant major depressive disorder
  • Documented inadequate response to multiple oral antidepressants (often at least two) during the current episode
  • Spravato used together with an oral antidepressant
  • Given at a REMS-certified clinic, with at least two hours of monitoring after each dose
  • No active substance use disorder or other listed contraindications

Different criteria apply to depression with acute suicidal thoughts. If you are in crisis, call or text 988.
